目的 探讨重症监护室多发伤患者发生下肢深静脉血栓的影响因素，为预防多发伤患者发生下肢深静脉血栓提供参考。方法 选取我院重症监护室收治的157例多发伤患者作为研究对象，根据住院期间是否发生下肢深静脉血栓，将患者分为下肢深静脉血栓组(n=28)和非下肢深静脉血栓组(n=129)。采用Mann-Whitney U检验、卡方检验对两组相关病历资料进行单因素分析，采用logistic回归模型分析下肢深静脉血栓的影响因素。结果 单因素分析显示，总住院天数、昏迷、WBC、CRP、INR、DFR、低蛋白血症等变量在多发伤患者是否发生下肢深静脉血栓方面存在差异(P均<0.05);logistic回归分析发现，WBC、DFR、低蛋白血症是多发伤患者发生下肢下肢深静脉血栓的独立影响因素(P均<0.05)。结论 临床护士应及时、动态监测多发伤患者的WBC、DFR、低蛋白血症情况，提供预见性护理措施，预防多发伤患者下肢深静脉血栓的发生。

Objective To investigate the influencing factors for lower extremity deep venous thrombosis in patients with multiple traumas, with the aim of informing prevention strategies.Methods The case data of 157 patients with multiple traumas admitted to the intensive care unit of our hospital were collected as the research subjects.According to whether lower extremity deep venous thrombosis occurred during hospitalization, the patients were divided into the lower extremity deep venous thrombosis group(n=28) and the non-lower extremity deep venous thrombosis group(n=129).Mann-Whitney U test and chi-square test were used to conduct univariate analysis on the relevant medical records of the two groups, logistic regression model was used to explore the influencing factors of lower extremity deep venous thrombosis of patients with multiple traumas.Results Univariate analysis showed that variables such as total hospital stay, coma, WBC,CRP,INR,DFR,and hypoproteinemia had differences in whether lower extremity deep venous thrombosis occurred in patients with multiple traumas(all P<0.05).The logistic regression analysis revealed that WBC,DFR,and hypoproteinemia were independent influencing factors for lower extremity deep venous thrombosis in patients with multiple traumas(all P<0.05).Conclusion Clinical nurses should dynamically monitor the WBC,DFR and hypoproteinemia of patients with multiple traumas, and implement predictive nursing measures in time to reduce the incidence of lower extremity deep venous thrombosis in patients with multiple traumas.
